63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was conducted today from 11:30am-1:20pm. There were 7 children (20 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, participating in a spanish immersion program, building with blocks, reviewing flash card, singing and dancing and eating lunch. Lunch served today was chicken soup with pasta and vegetable. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-170-A Description: Based on documentation review, the provider did not obtain a tuberculosis (TB) tests or screenings for each household member. Evidence: 1) Household member 3 does not have documentation of a tuberculosis test or screening prior to moving into the home in November 2018. Plan of Correction: I will tell them to get the tb test or screening.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each caregiver and household member every two years. Evidence: 1) Staff 2 has a tuberculosis test last conducted in 2/2017. 2) Household member 1 has a tuberculosis test last conducted on 3/15/17. 3) Household member 2 has a tuberculosis test last conducted on 2/24/17. Plan of Correction: I will make sure the tests are updated.
Standard #: 63.2-1720.1-B-2 Description: Based on record review, all staff did not submit to fingerprinting and does not have a completed criminal history record check. Evidence: Staff 1 does not have a completed criminal history record check. Staff 1 did submit to fingerprinting on March 26, 2019 but does not have results. (Staff 1, Hire date: 3/11/2019) Plan of Correction: I will follow up with the fingerprints.
