Provided a brochure of 3 hours of online training available. Your license expires in July 2019. Please submit your application prior to the expiration of the license.
An unannounced monitoring inspection was conducted this date. A total of 11 children (25pts) with Provider and 2 assistants present. When the inspector arrived, the children were seated waiting for lunch to begin. Lunch today was chicken, rice and peaches with milk to drink. Ample amount of food for second servings when requested. The staff were positioned sitting with the children while they ate and assisted with those children who could not feed themselves independently. Hand washing was observed after lunch and a nice transition to the carpet area for circle time, dancing and singing nursery rhymes was observed. Staff was observed sanitizing the tables, chairs and high chairs after lunch. Observation of the crib space was made. A total of 5 children's records, all staff and household records were reviewed. Previous violations were reviewed and corrective actions were verified. This inspection began at 11am and ended at 12:30pm. Thank you for your cooperation today. Standard #: 22VAC40-111-780-B Description: Based on child record review, one out of 5 records reviewed did not have an annual review of emergency contact information. Evidence: The last documented review of emergency contacts for Child #2 was dated 2/2018. Plan of Correction: I will review this with the parent today.
Standard #: 22VAC40-111-90-C Description: Based on review of children's records, updated immunizations were not obtained for one child. Evidence: Child #1 is under the age of 2. Last documented immunizations found was in November 2017. Immunizations are required every six months for children under the age of 2 years. Plan of Correction: I will request updated immunizations from the parent and file in the record.
