Technical Assistance:
Reviewed the requirements for an additional assistant caregiver to work with the children while the provider is not able to perform child care duties.

Comments:
An unannounced monitoring inspection was conducted today. Due to personal reasons, the provider is currently unable to perform in the role of child care provider/licensee. The substitute caregiver is currently working in the role of provider. The substitute caregiver was caring for 5 children, totaling 13 points. Children's records, the physical plant, children's nutrition, the care of children, caregiver records, and caregiver training records were inspected today. It was reported to the Inspector that no children's medications were on site today. The children were observed playing with toys, listening to stories, and singing/dancing to music. The caregiver displayed extreme patience in interacting with one child who was experiencing significant separation anxiety. This inspection was completed between approximately 9:45am and 11:15am. Today's exit interview was conducted with the substitute caregiver. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records are not complete. Evidence: Child records A, B, and C do not include the parent's medical insurance information.

Plan of Correction: I will ask each parent at pick up time.

Standard #: 22VAC40-111-140-D
Description: Based on interviews and a review of administrative documents, it was determined that the substitute provider has not kept a record of her time of arrivals and departures. Evidence: It was reported to the Inspector that caregiver B has acted in the role of substitute caregiver beginning yesterday, April 15, 2019. This caregiver did not document the time she worked in the role of a substitute.

Plan of Correction: I updated the documentation immediately during the inspection. I will keep an accurate record on the substitute sign in sheet.

Standard #: 22VAC40-111-530-C
Description: Based on interviews, it was determined that the children have not had crib linens assigned for individual use. Evidence: It was reported to the Inspector that two cribs are shared by two enrolled children who do not nap at the same time. The provider allows child A to use the crib for the morning nap, and child B uses the crib for afternoon nap. It was reported to the Inspector that the sheets were not changed each time in between crib uses.

Plan of Correction: I will remember to change the sheets in between each use if the children share the crib.
