An unannounced monitoring inspection was conducted today. There are 4 children (including one resident child) present (13 points) with 2 caregivers. 5 adult and 4 children's records were reviewed. See violation notice form missing information. Children were observed playing with a variety of toys today. E,ergency escape drills, first ad supplies, emergency supplies. and the menu are all in compliance. A Subsidy Heath and Safety Inspection (SHSI) was also conducted today.
Standard #: 22VAC40-111-60-B Description: Based on a review of records, children's records do not include all required information. Evidence: 3 children's records were reviewed. Missing information includes: *Parent information (father) (Children 2,3) *Second emergency contact. (Child 2) Plan of Correction: I will ask the parent to provide the missing information.
Standard #: 22VAC40-111-140-B-4 Description: Based on a review of records, no caregiver present is current in First aid certification. Evidence: 2 of 2 caregiver records reviewed did not contain documetnation of current first aid certification. the certification available for review expired 2/18/19. Plan of Correction: Both caregivers will get new certifications for first aid and CPR.
Standard #: 22VAC40-111-150-D-1 Description: Based on a review of records, there is no caregiver present current in CPR certification. Evidence: 2 of 2 records reviewed, did not contain current certification in CPR. The certificates available expired 2/18/19. Plan of Correction: Both caregivers will get new certifications for first aid and CPR.
Standard #: 22VAC40-111-320-B Description: Based on direct observation, child resistant covers were not in place in two outlets. Evidence: Two outlet were uncapped in the day care area next to children's cots. Plan of Correction: Outlets were recapped during the inspection.
