63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was conducted today. There were nine children in care ages five months to four years old. The provider and two additional caregivers were present during the inspection. Children, caregiver and household records were reviewed. Required postings, emergency evacuations, first aid kit and outside play were reviewed. Violations regarding children's records and background checks were identified and appear on the violation notice. These violations were reviewed with the provider at the conclusion of the inspection.
Standard #: 22VAC40-111-60-B Description: Based on record review and interview the provider failed to ensure that children's records contain all required information. Evidence: 1 - The records for children 1, 2 and 3 do not contain documentation of annual updates. a. child 1 has a documented first date of attendance as 1/11/18. There is no additional dated update. b. child 2 has a documented first date of attendance as 1/23/17 and the last information update is dated 2/28/`8. c. Child 3 does not have a documented first date of attendance. The enrollment application is dated 12/21/17. 2 - The provider confirmed that the records for child 1, 2 and 3 are past due for information updates. Plan of Correction: All children's records will be updated. Information updates will be clearly documented in each child's record.
Standard #: 63.2-1721.1-B-2 Description: Based on record review and interview the provider failed to ensure that all household members submit to fingerprinting. Evidence: 1 - One household member does not have documentation of fingerprint background checks on file in the family day home. 2 - The provider stated that she was unaware that the household member's current criminal history background check was no longer valid. Plan of Correction: The provider will ensure that the household member submits to fingerprinting as soon as possible.
