Technical Assistance:
Discussed age-appropriate feeding practices. Discussed proper storage of all food that children bring from home. The provider would like to be licensed for the age range of birth - 5 years. The provider shall no longer use the back deck for outdoor play time. Instead, the provider will begin walking the children to the neighborhood open play area or using the front driveway play space. In order to walk to the open play area, the provider shall obtain written permission from all parents. The provider shall begin using the child size table and chairs for children during meal and snack times.

Comments:
An announced full initial inspection for licensure was conducted today. The provider was caring for 4 children, totaling 8 points. Children's records, adult/caregiver records, background checks, health records, administrative documents, children's nutrition, the care of children, emergency preparedness items, and the physical plant were inspected today. The provider reported that no children's medications were on site today. The children were observed playing with toys, building with blocks, and eating morning snack from home. This inspection was completed between approximately 10:00am and 12:00pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on a review of emergency preparedness items, it was determined that the first aid kit is not complete. Evidence: The first aid kit does not include a thermometer and a second triangular bandage.

Plan of Correction: I will buy both things for the first aid kit.

Standard #: 22VAC40-111-800-A
Description: Based on a review of emergency preparedness items, it was determined that the emergency plan has not been completed as required. Evidence: The provider has not indicated a clear and distinct sound in the "method to alert" section of each emergency scenario.

Plan of Correction: I will update the plan.

Standard #: 22VAC40-111-110-B
Description: Based on a review of children's health records, it was determined that one immunization record was not completed as required. Evidence: The shot record for child A does not include the physician's signature on the statement indicating the child is adequately immunized for his/her age.

Plan of Correction: I will ask the parent to get the statement signed by the doctor.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record is not complete. Evidence: Child record C does not include the full name for one emergency contact person.

Plan of Correction: I will ask the parent for the information today.

Standard #: 22VAC40-111-690-D
Description: Based on observations and interviews, it was determined that the provider has not been sanitizing the diaper changing surface as required. Evidence: The provider has not been sanitizing the diaper changing pad in between each use.

Plan of Correction: I have been spraying the diaper pad with soap and water. I will start sanitizing it with a bleach/water solution in addition to the soap/water.
