Technical Assistance:
Technical assistance was provided regarding contacting zoning to increase your capacity and any children over the age of 18 living in the home must have all background checks and a TB screening.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-11:00am. There were 5 children (13 points) supervised by 2 direct caregivers. The home, programming, staff records, children?s records, household member?s records, evacuation drills, and policies were inspected. There were no medications on site today. Children were observed singing, reading and eating snack. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-111-170-A
Description: Based on review of household members records, one record is missing documentation of a report of Tuberculosis (TB) Screening. Evidence: Household member #1, turned 18 and does not have documentation of a TB screening.

Plan of Correction: I will get a record of her TB screening.

Standard #: 22VAC40-111-320-G
Description: Based on review of household records, there is no documentation of an annual fireplace/chimney inspection. Evidence: The current inspection for the fireplace was more than one year ago, 12/13/17.

Plan of Correction: I will have it inspected and make sure to do it every year.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of household members records, one record does not have documentation of a completed sworn statement. Evidence: Household member #1 turned 18 and has not completed a sworn statement.

Plan of Correction: I will have her sign a sworn statement.

Standard #: 63.2-1721.1-B-2
Description: Based on review of household members record, there is no documentation of an eligible fingerprint. Evidence: Household member #1, turned 18 and has not completed a fingerprint check.

Plan of Correction: She will get it done.
