Comments:
A announce monitoring inspection was conducted with the provider and staff. Ten children were in care with an age range of six months to four years. Total number in points was 32 points. Children were observed during rest time. Three house hold, two staff and six children's records were observed. if you have any questions about this inspection please call 540-272-9214 or e-mail lori.hancock@dss.virginia.gov. Inspection time 1:45pm to 3:30pm.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of three house hold member's records not all had a current T.B. Evidence: House hold member A's T.B test is dated 2/18/16.

Plan of Correction: They will have a new test done by next.

Standard #: 63.2-1720.1-B-2
Description: Based on review of three staff records not all had fingerprinting done before starting employment. Evidence: Staff member B with a start date of 2/1/18 , did not have a finger print check done before starting caring for children.

Plan of Correction: Staff B was sent home and will not return to work until the finger print check is back.
