Comments:
An unannounced monitoring inspection was completed today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 12:00-1:40pm. There were 7 children, totaling 15 points, under the supervision of 2 caregivers. Today, the children had circle time, colored pictures, works on making crafts for Easter, played outside, ate lunch and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken nuggets, peas and carrots, salsa, mixed fruit and milk. The Provider is currently certified in MAT (medication administration training). Description: Based on file review, 3 out of 5 children's records have not been updated annually. Evidence 1. Children #1 and #2 have records, which were was last updated on 1/10/18. 2. Child #3's record was last updated on 1/1/18. 3. Child #4's record was last updated on 10/10/17. 4. Child #5's record was last updated on 3/1/18.

Plan of Correction: These will be updated.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 5 children's records does not have additional immunizations updated every 6 months up the age of 2 years. Evidence 1. Child #3 is 2 years old. However, the most recent immunization record was completed when the child was 1 year old.

Plan of Correction: Next week will be this child's last day.

Standard #: 22VAC40-111-180-A
Description: Based on file review, 1 out of 3 caregiver/household members does not have a TB test/screening completed every two years. Evidence 1. Caregiver/household member #2's most recent TB test/screening was completed on 7/5/16.

Plan of Correction: An appointment has been made to have this completed tomorrow.
