Comments:
A complaint investigation was initiated on April 11, 2019 regarding allegations of ratios, diapering practices, safe sleep, discipline, and business practices. Interviews were conducted, and documentation was reviewed. There was not a preponderance of evidence to support the allegations regarding ratio, safe sleep or discipline; however, the complaint was deemed valid with regard to diapering practices, and business practices. The complaint related violations and non-related violations are identified in the Violation Notice. Complaint related: Yes
Description: Based on observation and interview with Provider, when meals or snacks are provided by the family day home, the menu for the current one-week period is not given to parents or posted or placed in an area accessible to parents. Evidence: Provider stated she does not give menus to parents. The menu posted by the front door was not for the current week (4/1-4/5).

Plan of Correction: The printer is fixed and the menus will be posted

Description: Based on interview with Provider, Staff did not have a record for each caregiver. Evidence: Staff#4, who started work on April 1st, did not have any required documentation or file available for review.

Description: Based on observation, caregivers did not wash their hands after contact with bodily fluids, or before feeding children. Evidence: After changing a child's diaper, Provider was observed feeding Child#1 without washing her hands.

Description: Based on observation, a child was left unattended on a changing table during diapering. Evidence: While changing a child, the Provider was observed turning and stepping away from the diaper changing station to check the monitor behind her and then stepping back.

Description: Based on interviews and observation, when a child's diaper becomes wet or soiled, the child isn't be cleaned and changed immediately. Evidence: 1. The Inspectors arrived at 11:30am and left at 2:00 pm. Only 2 children were observed being changed during this inspection. Children were observed having lunch and being put down for nap without a diaper change. 2. Staff#2 stated they changed all 7 children after breakfast (approximately 8:30-9:00). Provider stated the children would be changed when they woke up from nap (approximately 2:30-3:00) Provider also stated they only change children 1-2 times a day.

Plan of Correction: We will start using the daily sheets for all children in diapers

Description: Based on interview with Provider, Staff were employed without the person's completed sworn statement or affirmation. Evidence: Staff#4, who started work on April 1st, did not have a signed sworn statement.
