Comments:
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 12 points. Children's records, care of children, children's nutrition, health records, caregiver training, emergency preparedness, and the physical plant were inspected today. The children were observed singing with music, playing with toys, and interacting with the caregivers. The children were to be served cheese pizza, sliced grapes, green beans, and milk for lunch later today. This inspection was completed between approximately 10:30am and 11:45am. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record B does not include the address for one emergency contact person.

Plan of Correction: I will remind the parent to provide the information at pick up today.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver health records, it was determined that the provider has not obtained a repeat TB health screening within the past 2 years. Evidence: The provider's TB screening expired on 1/27/19.

Plan of Correction: I will get another TB screening done next week.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one item has not been maintained a clean, operable condition. Evidence: The diaper changing pad has ripped and torn in various places.

Plan of Correction: I will buy a new pad this weekend.
