Comments:
An unannounced inspection was conducted today between 11:00am and 12:45pm. There were 4 children (7 points) with one caregivers. The children were observed returning from the playground, playing floor games, preparing for lunch, handwashing and having lunch. Lunch consisted of Chicken Pasta, peas, oranges and milk. The physical plant, staff/household/children's records, emergency procedures, emergency supplies, menus and attendance records were reviewed. Description: Based on record review and interview with provider, the provider failed to ensure that shelter in place procedures were practiced a minimum of twice per year. Evidence: According to the provider, she is waiting until this time of year (storm season) to practice shelter in place so that the children will make the connection with weather and shelter in place drills.

Description: Based on review of children's files, the provider failed to maintain up to date record at the family day home for each enrolled child. Evidence: Child #4's file did not have documentation of a physician's address and phone number and the file did not contain documentation of authorization for pick up.

Plan of Correction: I will get information from parents and put in file.
