Technical Assistance:
Discussion on general requirements for staff. All staff must: Be able to speak, read, and write in English as necessary to: a. Carry out assigned job responsibilities, and b. Communicate effectively with emergency responders Discussion on sleeping areas for infants. All infants must sleep in cribs.

Comments:
An unannounced 2 month inspection was conducted this date. A total of 4 children in care, one of which belonged to provider(12pts) with 1 Staff. Observation of the programming, lunch time and transition to nap time was observed. Two new children's records reviewed. No medications at this time. Observation of the diapering and crib area was made. Some violations found today. Thank you for your cooperation today. This inspection began at 12:30pm and ended at 2pm. Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, two records were incomplete. Evidence: Child #1 and #2's record was missing a second emergency contact. Child #1's record was missing a work address for both parents. Child #2's record did not have written authorization for emergency care.

Plan of Correction: I will obtain this information today from the parents.

Standard #: 22VAC40-111-540-B
Description: Based on observation of the children in care, an infant walker was used. Evidence: Child #3, age 11 months was observed in an infant walker during the licensing inspection.

Plan of Correction: I will no longer use the walker for any children in care.
