Comments:
An unannounced renewal inspection was conducted today. There were six children in care, including one resident child, ages 21months through four years old. The children present totaled 11 points. Children were observed watching TV, playing with toys and singing. Children and staff records were reviewed as well as required postings, emergency items and emergency drills. Violations regarding children's records were identified and appear on the violation notice.

Violations:
Standard #: 22VAC40-111-60-A
Description: Based on record review and interview the provider failed to ensure that each child's record contains all required information. Evidence: 1 - The record for child 1 does not contain documentation of annual update. The most recent information in the record is dated November 3, 2017. 2 - The provider confirmed that there was not a annual update documented in 2018.

Plan of Correction: The provider will obtain updated information from the parent as soon as possible.

Standard #: 22VAC40-111-70-A
Description: Based on record review and interview the provider failed to ensure that before the child's first day of attendance, parents are provided in writing with information as whether or not there is liability insurance on the family day home operation. Evidence: 1 - The record for child 2 does not contain any documentation of information regarding liability insurance on the family day home operation. The record for child 2 has documented first date of attendance as 11/26/18. 2 - The provider confirmed that the record of child 2 does not contain documentation of liability insurance.

Plan of Correction: The parent will be provided with the documents and the documents will be signed as soon as possible.
