Technical Assistance:
22VAC40-111-(11)-830-B Shelter-in-place procedures shall be practiced a minimum of twice per year.

Comments:
An unannounced monitoring inspection was conducted with the provider and assistant beginning at 9:50 AM and ending at 11:35 AM. There seven children were in care during the inspection. The children were ages 11 months, 13 months, 19 months, 2, 3, 4, and 6 years old for a total of 19 points. The children were observed playing with toys and during outside recreation time. Bathroom and hand washing procedures were observed. Five children's records were reviewed. The provider's record, one household member's record, and one staff record were reviewed and in compliance. One pet record was reviewed and in compliance. The emergency drill log was reviewed. The first aid kit, flashlight, and weather band radio were observed. There were no medications on site to be reviewed. The most recent emergency escape drill was documented as 3/11/2018. The provider and assistant have current First Aid and CPR training certification. The provider has a current Medication Administration Training (MAT) certification which is required to administer prescription medication. Standard #: 22VAC40-111-830-B
Description: Based on a review of documentation, the provider did not have documentation of shelter-in-place procedures being practiced twice a year as required. Evidence: Documentation of two shelter-in-placed drills being conducted in 2018 were not available during the inspection.

Plan of Correction: The provider stated that she conducted two shelter-in-place drills were conducted but the documentation was unable to be located during the inspection. The provider stated that she would record all shelter-in-place drills and maintain the documentation.

Description: Based on a review of documentation, the provider did not maintain records of emergency evacuation and shelter-in-placed drills for one year as required. Evidence: There was no documentation of emergency evacuation and shelter-in-place drills for the year of 2018.

Plan of Correction: The provider stated that she misplaced the drill log for 2018. The provider stated that she would maintain the emergency evacuation drill log for one year.

Standard #: 22VAC40-111-60-B
Description: Based on a review of documentation, not all of the children's records were complete. Evidence: Child A, start date 2/24/19, did not have name, address and telephone number of each custodial parent's place of employment.

Plan of Correction: The provider stated that the parents did not want to provide this information. The provider stated that she would make another request for the missing information from the parents.
