63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on April 23, 2019 between the hours of approximately 10:15 a.m. and 1:30 p.m. Twelve (12) children were enrolled and four children were in the care of two caregivers, on this date (8 points). Two additional children were in care before school. Five children's records and two caregivers' records were reviewed and/or updated. No medications were administered, as is the policy in this family day home. Facility documents (attendance record, emergency evacuation and shelter-in-place logs and rabies vaccination for cat), posted information (daily schedule, license, inspection documentation, emergency plan/diagram, emergency telephone numbers and menu) and safety equipment (first aid kit, flashlight, fire extinguisher and radio) were observed/reviewed. Refrigerator and freezer temperatures were checked. The lunch served on this date included ham, macaroni and cheese, strawberries, cucumbers, bread and milk. Program observation was completed. Activities observed included: outdoor play; indoor play with toys; restroom and handwashing procedures; lunch; and naptime. Adult/child interactions, and daily routines/transitions were observed. The posted daily schedule and menu were consistent with the activities and meal provided. Substantial compliance continues to be maintained; exceptions to compliance with the licensing standards were identified on the Violation Notice. Standard #: 22VAC40-111-100-A
Description: Based on review/update of five children's records and discussion with the provider on April 23, 2019, the program failed to obtain documentation of a physical exam report within 30 days after the first date of attendance. Evidence: 1. Review of child A's record, on this date, determined that documentation of a physical exam report was not on file within 30 days of initial attendance (date of initial attendance - 10/09/2018). 2. The provider confirmed the lack of a physical exam report for child A.

Plan of Correction: The child's parent will be contacted and a physical exam report will be obtained.

Standard #: 22VAC40-111-110-B
Description: Based on review/update of five children's records and discussion with the provider on April 23, 2019, the program failed to obtain a signed immunization record for one child. Evidence: 1. Review of child A's record, on this date, determined that the immunization record was not signed by a licensed physician, physician's designee or local health department official. 2. The provider confirmed the lack of signature on the immunization record.

Plan of Correction: The immunization record will be returned to the parent for signature.

Standard #: 22VAC40-111-80-A
Description: Based on review/update of five children's records and discussion with the provider on April 23, 2019, the program failed to obtain verification of age and identity within seven business days of one child's first day of attendance. Evidence: 1. Review of child A's record, on this date, determined that verification of age and identity was not on file (date of initial attendance - 10/09/2018). 2. The provider confirmed that verification of age and identity was not on file for child A.

Plan of Correction: The child's parent will be contacted and verification of age will be obtained.
