Comments:
The licensing inspector conducted an unannounced renewal inspection, at which play groups, story time, activities, and personal hygiene were observed. Children were observed; and staff and children files and other documentation were reviewed. Two (2) children, aged 2yr-3yr, totaling 4 points were found to be in care with the provider. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:30am and concluded at 9:45am.

Violations:
Standard #: 22VAC40-111-830-B
Description: Based on review of documentation and discussions with the provider, the provider failed to maintain compliance with this standard as required related to practiced shelter-in-place drills. Evidence: The provider had only practiced 1 of the required 2 shelter-in-place drills for 2018.

Plan of Correction: Two drills will be done and recorded each year.

Standard #: 22VAC40-111-60-B
Description: Based on review of child record the provider failed to maintain compliance with this standard as required relating to parental information. Evidence: The record of child #2, enrollment date 1/21/19, did not have: 1. The fathers work address, work phone number, or home address listed as required. 2. The mothers work address or work phone number as required. 3. There was only 1 of the required 2 emergency contacts listed. 4. The father was listed as not being able to pick up, but there was no official court documented material to support this as required.

Plan of Correction: The information will be completed for compliance as soon as possible.
