Standard #: 22VAC40-111-100-A Description: Based on a review of children's health records, it was determined that one record for a child who has been in care more than 30 days does not include a physical exam report. Evidence: Child A entered care on 9/17/18. This child's record does not include a physical exam report. Plan of Correction: I have asked the parent many times to bring the physical form. She said she will bring it today at pick up. I will follow up to make sure she brings it.
