Comments:
A Subsidy Health and Safety inspection was conducted on Wednesday, April 24, 2019 from approximately 11:20 AM until 2:00 PM to review supplemental health and safety requirements. In attendance were three non-resident children. The children were being supervised by one caregiver during the inspection. There was a total of six points based on the children?s ages. The children were observed playing outside, playing with blocks and a ball maze, washing their hands, eating lunch and resting at nap time. Lunch consisted of chicken nuggets, bread, peas, peaches and milk. The caregiver was observed engaging the children in conversation and providing assistance when needed. A walk through of the home was conducted with the provider. The child care areas were filled with supplies and toys for the children. During the inspection five children?s, two caregivers? and one household member's records were reviewed.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice. If you have any questions about this inspection, please contact the licensing inspector, Lynn Powers at (804) 662-9790.
