Discussed the outdoor play area and equipment. Discussed monitoring the nap mats for any tears in the nonabsorbent surface.
- Comments:

An unannounced monitoring inspection was conducted beginning at 1:10 p.m. and ending at 2:45 p.m. with the provider and assistant. There were 9 children in care ranging in age from 23 months to 5 years of age for a total of 19 points. A 23 month old and three 2 year olds were observed sleeping in the nap room in cribs. Preschool children were observed working on a project to create a cottontail rabbit puppet after learning about the Desert Cottontail Rabbit. After their project, children performed a puppet show. After the puppet show, children had rest time and watched an age appropriate television program on their nap mats. Emergency drills were reviewed. There have been no new children enrolled since the last inspection. The provider is Medication Administration Training (MAT) certified but no children have medications on site. Standard #: 22VAC40-111-50-A Description: Based on review of the daily attendance record, attendance was not recorded accurately for April 25, 2019. Evidence: On April 25, 2019 at approximately 1:20 p.m., there were nine children in care and only two children were marked on the attendance sheet. Plan of Correction: The attendance was updated during the inspection.
