Comments:
An unannounced monitoring inspection was conducted today between 9:15am and 11:30 am. There were 8 children (29 points) and three caregivers present. The physical plant was inspected. Provider/Household/Children's records, evacuation logs, menu's, attendance records, emergency supplies and emergency plans were reviewed. The children were observed playing games and reading stories with caregivers. Handwashing and diapering were also observed. Areas of non-compliance were cited in the Violation Notice. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider failed to ensure that bottles are labeled with the child's full name and date, if more than one infant is in care. Evidence: There was an unlabeled bottle on a cabinet in the feeding area.

Plan of Correction: I will label all bottles.

Standard #: 22VAC40-111-60-A
Description: Based on review of children's files, the provider failed to maintain up to date records at the family day home for each child enrolled. Evidence: 1) Child #3 and # 4's files did not have documentation authorization to pick up. 2) Child #4's file did not have documentation of a medical insurance policy number.

Plan of Correction: I will get information from parents.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the provider did not ensure that no soft bedding of any kind will be used under or around infants including but not limited to quilts, comforters or stuffed toys. Evidence: An infant sleeping in a crib, had a stuffed toy in the crib.

Plan of Correction: We will not put soft toys in cribs.
