54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
An announced initial inspection was conducted today from 2:00pm-3:30pm. There were 3 children (10 points) directly supervised by 1 staff. The home, 1 staff record, 3 children?s records, 5 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, watching children's music videos and eating snack. There was a supply of books, toys, and materials for the children. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's full name and date. Evidence: There was more than one infant in care. 2 bottles were observed without a date. One bottle without a name or date label.

Plan of Correction: I will label the bottles with a name and date.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not maintain complete children's records. Evidence: 1) Child 1 is missing two emergency contacts. 2) Child 3 is missing one emergency contact.

Plan of Correction: I will ask the parents today for the information.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized. Evidence: Child 2 did not have documentation that the child has been adequately immunized.

Plan of Correction: I have asked the parents for the documents. I am waiting and will ask them again.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain from each adult household member a current report of tuberculosis screening form or a form documenting the absence of tuberculosis in a communicable form. Evidence: 1) Household member 1 and 2 have screenings dated from 2010.

Plan of Correction: We will get the records updated.

Standard #: 22VAC40-111-540-B
Description: Based on observation, walkers were in use by children. Evidence: Two children were observed utilizing two walkers.

Plan of Correction: The walkers were removed from the family day home area immediately. We will not use the walkers anymore. They will be put in the garbage.
