An unannounced renewal inspection was conducted on 04/26/2019 from 10:55 A.M-2:00 P.M. There were three children in care with the provider for a total of six points. The records for four children, one provider, one household member, evacuation drills, chimney inspection, emergency plan, outside play area, first aid supplies, annual training hours, and required posted information were reviewed. Children were observed during TV time, free play choice activity, story time, handwashing, lunchtime, and naptime. The exceptions to compliance are noted on the violation notice. Standard #: 22VAC40-111-100-A Description: Based on review of children records, the provider failed to obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #4's physical examination was dated 10/25/2017. Date of enrollment was 08/17/2017. 2. Staff verified the date the physical examination was received and the date of enrollment for Child #4. Plan of Correction: In the future the provider will track and communicate with the parent the time frame of when the physical examination is needed.
