- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, April 11, 2019 from approximately 10:20am to 12:00pm to determine the provider's compliance with licensing standards. During this inspection, there were 7 children in care totaling 24 points being cared for by two caregivers. The children were observed by the licensing inspector in their designated areas in the home, interacting with peers, playing with age appropriate toys, and watching television. Later, the children were observed taking bathroom breaks, washing hands, and preparing for lunch. The areas of childcare, kitchen, and the indoor play area were inspected. The required postings were reviewed and found to be in compliance. The first aid kit and emergency supplies, daily attendance, and fire and shelter-in-place drills were reviewed. Transportation is provided and one vehicle was inspected today. During the inspection, five children's records, the provider's records, one assistant's records, and one household member's records were reviewed. Standard #: 22VAC40-111-800-B Description: Based on record review on 04/11/2019, the provider did not ensure to review the family day home's emergency plan at least annually and update as needed. Evidence: 1) The licensing inspector observed the last documented review of the family day home's emergency plan was dated 04/2017. Plan of Correction: The provider will review and update the emergency plan as needed.
Standard #: 22VAC40-111-100-A Description: Based on a review of five children's records and interview on 04/11/2019, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to one child's attendance or within 30 days after the first day of attendance. Evidence: 1) The record for Child #4 (DOE: 02/26/2019) did not contain documentation of a physical examination. 2) During interview, Provider #1 confirmed documentation of Child #4's physical examination was not on file. Plan of Correction: The provider will request the documentation from the parent and ensure physical examinations are submitted timely.
Standard #: 22VAC40-111-430-A Description: Based on observation on 04/11/2019, the provider did not ensure that children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs. Evidence: 1) During the inspection, the main staircase by the entrance of the family day home was observed. The children in care were observed using the bathroom next to the staircase and there was no barrier to prevent the children access. Plan of Correction: The provider will ensure the barrier is placed back on the main staircase by the entrance of the family day home.
Standard #: 22VAC40-111-600-B Description: Based on observation on 04/11/2019, the provider did not ensure infants and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. Evidence: 1) During the inspection, the licensing inspector observed Child #1 (15-months-old) sitting in a high chair watching television from the beginning of the inspection at 10:20 am until 12:00pm when the inspection concluded. Plan of Correction: The child was removed from the high chair when the inspector left the inspection. The provider will remind the parent that children are not to have more than 30 minutes of consecutive time in a high chair or other confining piece of equipment. The provider will ensure this does not happen again.
Standard #: 22VAC40-111-690-E Description: Based on observation and interview on 04/11/2019, the provider did not ensure that disposable diapers shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper touches the exterior surface of the storage system during disposal. Evidence: 1) During the interview, Provider #1 indicated they lift the lid of the garbage can in the diapering area when disposing diapers. Plan of Correction: The provider will obtain the proper storage system for disposing diapers.
