Comments:
A Subsidy Health and Safety inspection was conducted on 4/26/2019 to review supplemental health and safety requirements. A total of 7 children and 3 resident children were present (23 points) with 2 caregivers. The children were engaged in the following: eating snack, listening to music, and playing with toys. Programming, emergency procedures, and 5 children?s records and 2 staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, the provider has not maintained a written hard copy of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The most recent daily attendance record was dated 3/15/2019. 2. Attendance for March 2019 was reviewed and arrival and departure times were not documented.

Plan of Correction: The provider created a daily attendance sheet to include arrival and departure times to be completed starting today.

Standard #: 22VAC40-665-410-A-3
Description: Based on staff interview, lockdown drills have not been practiced at least annually. Evidence: 1. The provider stated she was unaware of the requirement to complete a lockdown drill once a year and has not done so.

Plan of Correction: I just made a form to document the drills and I will complete one once a year.
