An unannounced monitoring inspection was conducted from 11:10am to 12:45pm. There were 5 children (15 points) with 3 caregivers. The children were observed sponge painting, playing outside and eating lunch. Hand washing before lunch was observed. The lunch menu included chicken nuggets, blueberries, carrots, veggie chips and milk. Emergency drill records were reviewed. Areas of non compliance are listed in the violation notice. Description: Based on record review, the provider did not practice evacuation drills monthly. Evidence: A review of the emergency drill record indicated that evacuation drills for the month of January 2019 and March 2019 were not practiced.

Plan of Correction: I will practice drills monthly and document on the log.

Standard #: 22VAC40-111-140-B-3
Description: Based on record review, the provider did not have a current certification in cardiopulmonary resuscitation (CPR). Evidence: The provider's CPR certification expired on 1/21/19.

Plan of Correction: I will call today to schedule the training.

Description: Based on record review, the provider did not have a current certification in first aid. Evidence: The provider's first aid certification expired on 1/21/19.

Plan of Correction: I will call today and schedule the training.

Standard #: 22VAC40-111-320-G
Description: Based on staff interview, the provider did not maintain documentation of an annual inspection for a wood burning stove in the family day home. Evidence: The provider stated that the annual inspection was completed for the wood burning stove but the inspection report could not be found during the inspection.

Plan of Correction: I will find the report and keep with my records.
