Today an unannounced Monitoring inspection took place between 11:10am and 1:15pm. There were 11 children present with the provider and 1 assistant for a total of 31 points within the supervision guidelines. The children were observed counting to 17 and learning about the letter 'V' with worksheets, washing hands, eating lunch (meatballs with rice, fruit, mixed veggies and milk), being read to and napping. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Thank you for your cooperation. If you have any questions, please email me at email@example.com.
Standard #: 22VAC40-111-900-B Description: Based on observation, children were permitted to drink while walking and laying down. Evidence: 1. After lunch was finished the children were given sippy cups of milk. 5 out of 11 children were observed walking around the family day home drinking from their sippy cups. 2. At the start of nap time, child #6 was given a bottle of milk while laying down on their mat. Plan of Correction: I'll make it one of the rules that we can't get up with our milk until we're done.

Evidence: Household member #1's most recent TB screening is dated 4/21/2017. Plan of Correction: He will have a TB test soon.
