Discussed the following with the Provider: - sample forms to be used for children's registration - point system requirements for additional caregivers - emergency plan information

An announced initial inspection was completed today. The physical plant, 4 caregiver/household member files, caregiver training, child nutrition, emergency procedures and supplies were inspected between 2:00-3:15pm. There are no children enrolled to receive care at this time. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. Once the results of the search of the central registry are reviewed by the Licensing Inspector, a conditional license will be issued. Should you have any questions, please contact me at

Description: Based on observations, the first aid kit did not include all required items. Evidence 1. The First aid kit is missing scissors, tweezers, a digital thermometer and triangular bandages.
