Comments:
An unannounced monitoring inspection was initiated today. The physical plant, 2 caregiver/household member files, 2 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:20-2:45pm. There were 3 children, totaling 6 points, under the supervision of the Provider. During the inspection, the children were observed listening to music, enjoying free play, cleaning up for and leaving for the day. For lunch, the child was served chicken, rice, mixed vegetables, oranges and milk. The Provider is not certified in MAT (medication administration training). Standard #: 22VAC40-111-830-C
Description: Based on file review and interview, the Provider did not complete a minimum of 2 shelter-in-place drills each year. Evidence 1. On the evacuation drill paperwork, there was no documentation of a Shelter-in-place drill having been completed in 2018.

Plan of Correction: The Provider will do this each year.

Standard #: 22VAC40-111-920
Description: Based on observations, the posted menu did not include all required information. Evidence 1. The posted menu did not include the dates to correlate with what was being served.

Plan of Correction: The Provider fixed this during the inspection. She posted the menu for May during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on interview, 1 out of 3 children in care do not have a record on file. Evidence 1. Child #3 began care today. There is no record on file for this child.

Plan of Correction: The Provider spoke to the parent during the inspection. She will not allow the child return for care until the parent brings in the completed record.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 2 children's records reviewed does not have additional immunizations every six months up to the age of 2 years. Evidence 1. Child #2 is 25 months old. The most recent immunization record was updated when the child was 15 months old.

Plan of Correction: The Provider will ask the parents for this again.

Standard #: 22VAC40-111-210-A
Description: Based on file review, the Provider did not attend a minimum of 16 hours of training annually. Evidence 1. For 2018, the Provider attended 10 hours of training.

Plan of Correction: The Provider will do 16 hours this year.
