Comments:
An unannounced renewal inspection was conducted today between 9:15am and 12:30pm. There were 6 children (21 points) present with 2 caregivers. An inspection of the entire Family Day Home was completed. Caregivers/household members/children's records, emergency plans, cribs, evacuation logs and attendance records were reviewed. The children were observed playing with caregivers and having morning snack brought from home. Areas of non compliance were cited in the Violation Notice. Standard #: 22VAC40-111-830-A
Description: Based on a review of the emergency evacuation logs, the provider failed to ensure that emergency evacuation procedures will be practiced monthly with all caregivers and children in care. Evidence: There is no documentation for emergency evacuation drills for calendar year 2018. There is documentation for one emergency evacuation drill for 2019 - (04/11/2019)

Plan of Correction: I will do fire drill and write them down

Standard #: 22VAC40-111-60-A
Description: Based on children's record review, the provider failed to ensure that records were up-to-date for each child enrolled in the family day home. Evidence: 1) Child #2 and #3's files did not have documentation of a physical by the 30th day of attendance. 2) Child #3 and #4's files did not have documentation of a medical insurance policy number. 3) Child #5's file did not have documentation of an annual update.

Plan of Correction: I will get information from parents.

Standard #: 22VAC40-111-90-C
Description: Based on review of children's records, the provider failed to ensure that the family day home will obtain documentation of additional immunizations for a child who is no exempt from the immunization requirements every six months for children under 2 years old. Evidence: Child #4 (dob 6-16-18) most recent document immunizations 09/17/18.

Plan of Correction: I will get recent immunizations from parents.

Standard #: 22VAC40-111-210-A
Description: Based on review of training logs, the provider failed to ensure that caregivers obtained a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities. Evidence: The Provider did not have documentation of a minimum of 16 clock hours of training annually related to job responsibilities.

Plan of Correction: I will get additional hours of training.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the child care area were not maintained in a clean, safe and operable condition. Evidence: The cabinet door is broken and is falling off the hinges in the bathroom used by the children.

Plan of Correction: I will have it repaired.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider failed to ensure that child resistant protective covers will be installed on all unused electrical outlets accessible to children. Evidence: The electrical outlet in the entry, under the high chairs does not have a protective cover.

Plan of Correction: Outlet cover put in place during inspection.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the provider failed to ensure that high chairs will be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened. Evidence: Two children were observed sitting in high chairs without the safety strap securely fastened.

Plan of Correction: We will remember to use safety straps.
