Comments:
An unannounced renewal inspection was conducted today from 10:00am-11:45pm. There were 3 children (6 points) directly supervised by the provider. The home, 1 staff record, 4 children?s records, 2 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Medications records were in compliance. Children were observed playing freely, playing with legos, and learning about animals. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-830-C
Description: Based on record review, shelter-in-place drills were not practiced twice a year. Evidence: There was no documentation of shelter-in-place drills practiced in 2018.

Plan of Correction: We will practice and document all shelter-in-place drills.

Standard #: 22VAC40-111-90-C
Description: Based on record review, it was determined that the provider did not obtain additional immunizations once every six months for a child under the age of two years. Evidence: Child#2's record contains an immunization record that was last updated on 8/13/2018. The child was 12 months of age when the record was last updated.

Plan of Correction: I will ask the parents to bring an updated immunization record.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on record review and interview, the family day home did not obtain results from the child protective services central registry for a 14 year old residing in the home. Evidence: Household member#2's record does not contain a child protective services central registry check report. A person living in the home of a licensed family day home provider shall have a child protective central registry check within 30 days of a 14 year old beginning to reside in the home, or a person in the home becoming 14 years old.

Plan of Correction: We will send a new form to the agency, the last form was returned to us twice.
