Technical Assistance:
Technical Assistance was offered regarding the following: attendance records, emergency plan and high chairs.

Comments:
An unannounced monitoring inspection was conducted from 11:15am through 1:10pm. There were 11 children (26 points) with 4 caregivers present. A total of 4 children's files and 2 staff member's files were reviewed. The children were observed eating lunch, using the bathroom and getting diapers changed, hand washing, playing with toys and getting ready for nap time. The lunch menu included rice with veggies and turkey, cantaloupe and milk. One medication and emergency drill records were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-120-A
Description: Based on staff interview, the provider did not maintain a record for each caregiver. Evidence: 1. Caregiver # 2's first day of employment was 4/26/19. 2. Caregiver # 1 stated that she does not have any written record information for Caregiver # 2 including name, address, results of 2 reference checks, emergency contact information, verification of age, and first date of employment.

Plan of Correction: If Caregiver # 2 continues to work here then we will maintain a complete record.

Standard #: 22VAC40-111-170-A
Description: Based on staff interview, the provider did not obtain from each caregiver at the time of hire prior to coming into contact with children a current Report of Tuberculosis Screening (TB). Evidence: Caregiver # 1 stated that Caregiver # 2's first day of employment at the family day home was 4/26/19. Caregiver # 1 did not obtain a TB screening from Caregiver # 2 prior to Caregiver # 2 coming into contact with the children.

Plan of Correction: If Caregiver # 2 continues to work here then we will obtain a TB screening.

Standard #: 22VAC40-191-60-B
Description: Based on staff interview, a caregiver at the family day home did not complete the Sworn Disclosure Statement (SDS) prior to the first day of employment. Evidence: Caregiver # 1 stated that Caregiver # 2's first day of employment at the family day home was 4/26/19. Caregiver # 2 did not complete the SDS prior to her first day working with the children.

Plan of Correction: I will have her complete the form today.

Standard #: 63.2-1720.1-B-2
Description: Based on staff interview and observation, the family day home provider did not obtain fingerprint results for a caregiver before the first day of employment. Evidence: 1. Caregiver # 1 stated that Caregiver # 2's first day at the family day home was 4/26/19. Caregiver # 2 was observed providing care and supervision for the children present. 2. Caregiver # 1 stated that Caregiver # 2 is being observed for potential employment, and Caregiver # 1 did not think that Caregiver # 2 would need fingerprints for that.

Plan of Correction: I will make sure that no caregivers are employed without obtaining fingerprint results.
