Technical Assistance:
Relating to organizing paperwork for household members.

Comments:
An unannounced renewal inspection was conducted today between 9:30am and 11:45am. There were 4 children (9 points) with one caregiver. The children were observed eating snack brought from home, playing with puzzles and watching a video. The physical plant was inspected. Caregiver/household members/children's records, emergency supplies, attendance records, evacuation logs, and emergency plans were reviewed. Standard #: 22VAC40-111-60-A
Description: Based on review of children's records, the provided failed to ensure that records were maintained and up to date for all children enrolled in the family day home. Evidence: 1) Child #1's file does not have documentation of authorization to pick up, proof of immunizations on first day of attendance or proof of a physical by the 30th day of attendance. 2) Child #3's file does not have documentation of a medical insurance policy number or proof of immunizations on the first day of attendance or proof of physical by the 30th day of attendance.

Plan of Correction: I will get the information from their parents.
