Comments:
A Subsidy Health and Safety inspection was conducted today between 9:30am and 11:45am to review supplemental health and safety requirements. There were 4 children (7 points) with one caregiver, The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on record review, the provider failed to ensure that the family day home had written lockdown procedures. Evidence: The provider did not have documentation of a written emergency plan that included lockdown procedures

Plan of Correction: I will add lockdown to my emergency plan.

Standard #: 22VAC40-665-410-A-3
Description: Based on evacuation log review, the provider failed to ensure that lockdown procedures will be practiced once annually. Evidence: The provider did not have documentation of lockdown procedure practiced during the 2018-2019 year to date.

Plan of Correction: I will include lockdown drills and write them down.
