Comments:

An unannounced renewal inspection was conducted today between 12:00pm and 2:20pm. There were 7 children (19 points) present with three caregivers. The children were observed singing and playing with instruments, handwashing, eating lunch and preparing for nap. Lunch consisted of chicken, vegetables, rice, fruit, milk and water. The physical plant was inspected. Caregivers/household members/children's records, emergency procedures and plans, menus, attendance records and emergency supplies were reviewed. Description:	Based on review of children's records, the provider failed to ensure that records were maintained and up to date for each child enrolled at the family day home. Evidence: 1) Child #1's file did not have documentation of authorization for pick up or proof of immunizations on the first day of attendance. 2) Child #3's file did not have documentation of immunizations on the first day of attendance.
Plan of Correction:	I will ask the parent's for the information and put in the children's files.
