An unannounced monitoring inspection was conducted on April 11, 2019, from approximately 1:22pm to 3:23pm. Two staff present with 12 children ( 28 points). Children were observed in preparing for nap and sleeping. Sic children records and two caregiver records reviewed on this date. Exit interview conducted with staff in charge.
Standard #: 22VAC40-111-510-C Description: Based on observation and staff interview, the provider did not ensure cots , mats and cribs were at least 12 inches from each other. Evidence: During the inspection conducted on April 11, 2019, the licensing inspector observed 9 occupied cots that were between 3 and 7 inches apart. Staff confirmed the cots were not at least 12 inches apart. Plan of Correction: The provider and staff are planning move the room around and put away stack able chairs and tables during nap time.
Standard #: 22VAC40-111-590-A Description: Based on observation and staff interview, the provider did not ensure infants were placed o n their backs when sleeping or napping unless otherwise ordered by a written statement signed but the child's physician Evidence: During the inspection conducted on April 11, 2019, child #1 was sleeping on his stomach . Staff #1 stated child is laid on stomach when a sleep and confirmed child #1 cannot roll over. Plan of Correction: Staff #1 will put infants on there back when sleeping. Staff #1 moved infant on back when inspector present.
