Technical Assistance:
Licensing inspector discussed provisional license and child abuse and neglect training with provider.

Comments:
An unannounced renewal inspection was conducted today from approximately 12pm-2:20pm. There were 7 children (17 points) in the direct care and supervision of 2 caregivers. A sample of 4 children?s records, 2 caregiver's records and 3 household member's records were reviewed. The children were observed napping. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not annually review children's records with the parent. Evidence: Child C's file was last reviewed 01/11/2017.

Plan of Correction: I will do it today.

Standard #: 22VAC40-111-830-B
Description: Based on interview, shelter-in-place procedures were not practiced twice per year. Evidence: There was no documentation of practiced shelter-in-place procedures. The provider stated that she does not have any.

Plan of Correction: I will find it and send you a picture.

Standard #: 22VAC40-111-830-D
Description: Based on record review and interview, emergency evacuation and shelter-in-place drills were not maintained for one year. Evidence: The emergency evacuation and shelter-in-place records for 2018 were not available. The provider stated that she could not find them.

Plan of Correction: I will find it and send you a picture.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: 2 of 4 Children's records reviewed were missing required information. 1. Child A was missing the medical insurance policy number. 2. Child B was missing date of enrollment, medical insurance policy number and medication authorization form.

Plan of Correction: I will text the parents for information today.

Description: Based on observation, child resistant covers were not on unused outlets. Evidence: 3 unused outlets on the wall near the television were not covered with protective covers.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom was missing required contents. Evidence: The bathroom located in the main child care area did not have paper towels.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-510-C
Description: Based on observation, occupied cribs and mats were not at least 12 inches away from each other. Evidence: 1. Seven children were sleeping in one bedroom 2. Two children were sleeping in cribs that were touching, there was no space between the two occupied cribs. 3. Four children were sleeping on mats that were touching, there was no space between the occupied mats.

Plan of Correction: I will move them.

Standard #: 63.2-1721.1-B-2
Description: Based on record review, the provider did not submit to fingerprinting. Evidence: The provider did not have complete fingerprinting results on file.

Plan of Correction: I will send my appointment to you.
