Comments:

An unannounced monitoring inspection was conducted today from 11:00am-1:00pm. A total of 2 children [totaling 6 points] were present with 1 caregiver. Children were observed during organized activities, handwashing and lunch. A sample of 2 children's records, emergency drills and emergency supplies were reviewed. Standard #:	22VAC40-111-770
Description:	Based on observations, a working battery operated radio was not available during the inspection. Evidence: The battery operated radio was inoperable at the time of the inspection.
Plan of Correction:	I will try new batteries.
Standard #:	22VAC40-111-60-B
Description:	Based on a sample of records reviewed, 1 out of 2 children's records did not include the required information. Evidence: The record for Child #2 was missing a medical insurance and policy number and the parents' written authorization for emergency medical care.
Plan of Correction:	I will get the information from the parents.

Description:	Based on a sample of records reviewed, 1 out of 2 children's records did not include the required written information to parents prior to the first day of attendance. Evidence: The record for Child #2, who was observed in care, was missing policies for medication administration.

Description:	Based on a sample of records reviewed, 2 out of 2 children's records did not include documentation of verifying the child's age and identity within seven days of enrollment. Evidence: The records for Child #1 and #2 were missing documentation of age and identity verification.
