Comments:
An unannounced mandated Monitoring inspection was conducted today from 10:00am to 11:30 am. There were 11 children present (29 points) with 2 caregivers. Children were observed cleaning up from morning snack, doing yoga, playing outside, and having tunny time. Reviewed 2 staff files, 2 resident files, 5 children's files, attendance, emergency evacuation log, menus, playground and physical plant. Standard #: 22VAC40-111-100-A
Description: Based on record review, provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child#2 did not have documentation of a physical. Start date 3/4/19.

Plan of Correction: I will ask the parent for the physical.

Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not provided provisions of the family day home's emergency preparedness and response plan. Evidence: Child#2 did not have signed documentation of parental receipt of the family day home's emergency preparedness and response plan.

Plan of Correction: I will give to the parent

Standard #: 22VAC40-111-180-A
Description: Based on record review, provider did not obtain current TB screenings every two years from the date of the first screening for caregivers. Evidence: Staff#1 and #2 had expired TB screenings (February 2017- expired Feb 2019)

Plan of Correction: We will go get new ones done
