An unannounced mandated Monitoring inspection was conducted today from 10:00am to 11:30 am. There were 11 children present (29 points) with 2 caregivers. Children were observed cleaning up from morning snack, doing yoga, playing outside, and having tunny time. Reviewed 2 staff files, 2 resident files, 5 children's files, attendance, emergency evacuation log, menus, playground and physical plant. Standard #: 22VAC40-111-100-A Description: Based on record review, provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
