Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the background check (CPS) for Staff #3.

Comments:
The following inspection took place between the hours of 12 pm (noon) through 1:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 11 children (31 points) with 3 caregivers. A total of 4 new children?s files, 3 staff files, and 1 household member?s file was reviewed. The children were engaged in the following: finishing lunch time, music, toy play, diapering, nap time, bathroom break, hand washing, and other organized activities. Standard #: 22VAC40-191-60-C-2
Description: Based on review, 1 of 3 staff files reviewed indicated that a staff person did not have a central registry finding (CPS check) within 30 days of employment. Evidence - 1. On the date of inspection (04/30/2019), an original CPS check for Staff #3 was not available for review. 2. The start date for Staff #3 was listed as 11/14/2019. The Provider has expressed difficulty in the past of obtaining this background check due to it being sent back for missing information, or insufficient funds. It appears that the copy on file was notarized on 04/09/2019. The Provider stated (during an interview conducted on the date of inspection) that the original was sent again on the date of notarization. 3. On the date of inspection, the Office for Background of Investigation (OBI) was contacted by the Licensing Inspector (LI), their representative stated that their office has no record of this request.

Plan of Correction: All future employment of Staff #3 shall be denied. A new request of the central registry shall be sent immediately (within 10 days. Once the finding has been received, it shall be shared with the LI. Once the LI has reviewed this new documentation, then Staff #3 shall return.
