Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a discussion of the process if the Family Day Home should relocate to a new home in the future; a modification would need to be requested, and new form submitted, to include but not limited to a Contact with the Local Zoning Administrator form.

Comments:
The following inspection took place between the hours of 10 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (24 points) with 2 caregivers. A total of 9 children?s files, 2 staff files, and 1 household member?s file was reviewed. The children were engaged in the following: toy play, movie time, music, song, diaper changing, nap time, hand washing, games, getting ready for lunch, and other organized activities. Standard #: 22VAC40-111-60-A
Description: Based on review, 1 of 9 children's files reviewed indicated that the Provider did not maintain an up-to-date record at the Family Day Home (FDH) for each enrolled child. Evidence - 1. On the date of inspection (05/01/2019), a children's record was not available for review for Child #6. 2. This documentation was missing from the file of Child #6.

Plan of Correction: This documentation shall be completed and placed on file for Child #6.

Standard #: 22VAC40-111-60-B
Description: Based on review, 4 of 9 children's files reviewed indicated that not all of the children's records contained all of the required information/documentation. Evidence - 1. On the date of inspection (05/01/2019), the following information/documentation was not available for review: a. The child's Insurance policy name and policy number, for - Child #7 b. Information for Parents form, for - Child #6, and Child #7 c. Policy for the Administration of Medication form, for - Child #6, Child #8, and Child #9 d. Provisions of the Emergency Preparedness and Response Plan form, for - Child #6 e. Liability Insurance Declaration form, for - Child #6, and Child #9 2. All listed documentation/information was missing from the applicable child's file.

Plan of Correction: All missing documentation/information shall be obtained and placed on file.

Standard #: 22VAC40-111-90-C
Description: Based on review, 1 of 9 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child who was not exempt from the immunization requirements according to subsection B of this section once every six months for children under the age of two years. Evidence - 1. On the date of inspection (05/01/2019), the immunization documentation on file for Child #6 was dated - 06/01/2018. 2. On the date of inspection, Child #6 was under the age of two years.

Plan of Correction: An updated immunization page shall be obtained and placed on file for Child #6.
