Technical Assistance:
Updated subsidy requirements and training discussed. Written information left with vendor regarding these changes.

Comments:
An unannounced subsidy health and safety inspection was conducted on 05/02/2019 from 11:11am - 12:12pm. A monitoring inspection was additionally conducted as this home is a licensed family day home. At the time of entrance there were six children in care with two caregivers. The ages of the children counted for a total of 11 points. The children were observed playing with toys in the child care room with the assistant. Records were reviewed for two caregivers and one household member. Records for four children in care reviewed.

Violations:
Standard #: 22VAC40-665-160-B
Description: Based on record review, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: Two of six children in care were not listed on the attendance/sign-in record for the family day home when the inspection arrived at 11:11am.

Plan of Correction: The provider stated that these two children arrived just prior to the inspector and were dropped off during outdoor play. Both children were noted in the attendance sheet during the inspection. Going forward all children will be noted on the attendance sheet when they arrive.

Description: Based on record review, the vendor failed to included in the written emergency plan for the family day home continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: The written emergency plan for the family day home did not contain procedures for continuity of operation.

Plan of Correction: The emergency plan will be updated to include continuity of operations.
