An unannounced monitoring inspection was conducted today. There are 12 children (42 points) with 3 adults. Children were observed playing with blocks, resting quietly and singing. 2 adult and 2 children's records were reviewed. See violation notice for missing information. Emergency escape drills, emergency supplies, fridge temperature, first aid supplies, menu and attendance were all in compliance. Standard #: 22VAC40-111-170-A Description: Based on a review of records, one staff record did not contain documentation of a current TB screening at time of hire. Evidence: Staff B - (DOH: 1/1/19) TB screening is dated 3/6/2019. Plan of Correction: Staff will provide a TB test result when hired.
Standard #: 22VAC40-191-60-B Description: Based on a review of records, one staff record did not contain a completed Sworn Disclosure statement prior to employment. Evidence: Staff B (DOH: 1/1/19) - Sworn Disclosure statement is dated 1/20/2019. Plan of Correction: Forms will be signed at date of employment.
Standard #: 63.2-1720.1-B-2 Description: Based on a review of records, staff did not submit to fingerprinting prior to employment. Evidence: Staff A: (Date of Hire: 1/15/2019) does not have a completed fingerprint search on file. Staff B: (Date of Hire:1/1/2019) fingerprint results are dated 1/29/2019 Plan of Correction: New staff will not be hired before fingerprinting is completed.
