Comments:
An unannounced renewal visit was conducted today between the hours of 9:15 am through 10:30am with the provider on site. There were 5 children (15 points) with the provider. A sample of 2 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: playing outside, toy play, diaper changes, hand washing and nap time. Interactions between the children and the provider were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, evacuation drills, attendance records, injury reports and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on record review, the provider did not obtain for each adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening. Evidence: 1. Household Member #1 had a tuberculosis screening form dated 3/01/2017.

Plan of Correction: I will set up an appointment for the household member.
