Thank you for your time and assistance with the unannounced monitoring inspection conducted today, May 7, 2019. The on-site inspection was held from 9:15 a.m. to 11:35 p.m. On this date, there were six children in the care of the provider (15 points). There were a total of six children enrolled in the program. There were five children's records reviewed, along with the records for the provider and one household member. The provider and two children were interviewed. There were no medications being administered at the time of the visit, as is the policy of the provider. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. The lunch being prepared was observed to meet USDA guidelines and consisted of cheese pizza, carrots, mandarin oranges, and milk. The children enjoyed working on special Mother's Day gifts. Some items reviewed/observed during the inspection included: first aid kit, emergency supplies, warm water temperature, refrigerator and freezer temperatures, required posted information, documentation of evacuation and shelter-in-place drills, choice time, hand washing and bathroom procedures, infant care, diaper changing procedures, art activities, yoga practice, circle time, and music and movement. Standard #: 22VAC40-111-60-B Description: Based on review of five children's records and verification from the provider, two children's records did not contain all of the required information. Evidence: 1. The record for Child #2 did not contain the name, address and phone number of a second person to contact in case of an emergency and the parent could not be reached. 2. The record for Child #1 did not contain the physical address of the second emergency contact the parent had named in case of an emergency and the parent could not be reached. 3. The provider confirmed the records for Child #2 and Child #1 did not contain required information. Plan of Correction: The provider will call the parents of the children today or talk with the parent at pick-up time and obtain the missing information.
