5/07/2019 Arrived 1:05pm Departed 2:30pm An unannounced monitoring inspection was completed today. Four children were in care. The children were watching television, playing with cars and eating a snack. You reported that you have ordered a new crib, but do not have any infants in care. Thank you for your cooperation. Description: Based on the review of five children's files, the facility filed to obtain updated immunization information. Evidence: 1. There is no updated information on file for child #3.

Plan of Correction: The provider will obtain the updated immunization record from the parent.
