A announced monitoring insepction was conducted with the provider and one assistant. There were nine children in care with a total number of points 22 . The children were observed during rest and free play. No children were on medications. two staff files and seven children's records were reviewed. The provider will be closed August 19th-23rd. Standard #: 22VAC40-111-830-A Description: Based on review and interview with the provider there was no documentation of monthly escape practices. Evidence: There was no monthly escape practices documented for 2019. Plan of Correction: I will complete the drills and document every time we conduct a drill.
Standard #: 22VAC40-111-60-B Description: Based on review of five children's records not all were complete. Evidence: Child A with a start date of 12/16/18 was missing the address for physical and physical. Child B with a start date of 9/14/17 was missing a physical and the insurance policy number, Child C 10/23/17 was missing a physical. Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-111-570-A Description: Based on observation and staff interviews it was determined that assistant provider was caring for 22 points alone. Evidence: When LI arrived the provider returned home and stated that she went to the bank leaving assistant A alone with nine children totaling 22 points. The provider stated she was gone for 10-15 minutes. Plan of Correction: I will not leave the day care when the points are over 16.
