Technical Assistance:
Discussed: outdoor play equipment, staff records, children's records

Comments:
An unannounced monitoring inspection was conducted beginning at 10:20 a.m. and ending at 12:30 p.m. with the provider and two assistants. There were 12 children in care ranging in age from 6 months to 3 years for a total of 31 points and the provider's two children for a total of 35 points under the care and supervision of 3 staff. Infants were observed napping. Preschool children were having outdoor play that included an activity about the theme of the week "flowers". After outdoor time, children had lunch. Handwashing procedures were observed. Lunch today was peanut butter and jelly sandwiches, corn, oranges, banana and milk. After lunch, children prepared for nap. Two infants were observed being fed bottles. Infants had play time while the preschool children napped. Four children's records, three staff records and one household member record were reviewed. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Standard #: 22VAC40-111-50-A
Description: Based on observation, the attendance record did not include all children in attendance. Evidence: On May 10, 2019, at approximately 10:25 a.m., there were nine children on the attendance sheet but twelve children were present.

Plan of Correction: Attendance was updated during the inspection. Parents will be reminded to mark their children in attendance and staff will double check to ensure all children in care are marked in on the attendance record.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, the provider failed to obtain central registry background checks within 30 days of employment. Evidence: Staff #2, start date 9/10/2018, did not have documentation of satisfactory central registry background check in the record.

Plan of Correction: I was under the impression that the new national fingerprint background checks took place of all previous background checks. In the future, central registry background checks will be requested within the first 7 days of employment.

Standard #: 63.2-1720.1-B-2
Description: Based on review of documentation and interview with staff, the provider failed to obtain national fingerprint background checks prior to the first day of employment. Evidence: Staff #2, start date 9/10/2018, fingerprint background checks is dated 9/20/2018.

Plan of Correction: In the future, all fingerprint background check results will be on file before starting.
