Technical Assistance:
Technical assistance was offered regarding obtaining central registry findings for household members when they reach the age of 14 years.

Comments:
An unannounced monitoring inspection was conducted from 11:05am to 12:20pm. There were 11 children (27 points) in care of 2 caregivers. The children were observed playing outside with sand and outdoor toys, washing hands after returning into the family day home, playing with toys in small groups, and being bottle fed. Caregiver to child interactions were observed to be positive, and age appropriate methods of behavior guidance were being used. 5 children's, 2 caregiver's and 1 household member's record were reviewed. Emergency drill records and staff training records were also reviewed. There were no medications on site. Areas of non compliance are listed in the violation notice. Description: Based on records review, the sworn disclosure statements (SDS) for caregivers and an adult household member were not updated every 5 years. Evidence: The SDS for Caregiver # 1, Caregiver # 2 and Household Member # 1 expired on 4/7/19.

Plan of Correction: I will update the forms and place them in each record.
