Technical Assistance:
Consultation was given on recalled Fisher Price baby rocker. A link to the CPSC web site and recall was forwarded to the provider.

Comments:
An unannounced monitoring visit was conducted today. Ten children were present with three caregivers for twenty eight points which is within points for supervision. Children were observed transitioning from morning snack and free play. Reviewed a sample of children's records, emergency preparation, physical space, trainings and personnel records. All three caregivers have 20 hours of training as of the visit. Lunch to be served today will be chicken with vegetables, quinoa, fruit and milk. Bottles for all babies are labeled. Description: Based on provider statement, written authorization to give an over the counter medicine was not completed by the parent. Evidence: No paperwork has been completed by the parent to authorize the provider to administer an over the counter cough medication to child #2.

Description: Based on observation, an over the counter OTC) medication was not labeled with the child's name. Evidence: A OTC medication that was present in the home today was not labeled with the child's name.
